This was the result of very poor workmanship. Client had a loose toilet paper holder. Did steps to remove it. Set screw completly out. Still not budge. They used these anchors and polyurethane glue. GLUED TO WALL can you believe it. A 2in job turned Into a 2 hr with mudding patching and painting. The bad part there was a wood block literally 1 inch away from where they mounted. Could have installed properly. Absolutely no excuse to use GLUE on a toilet paper holder.
